25 | On June 7, 2023 at 9:02am Licensing Program Analyst (LPA) Indira Loza arrived at the facility to conduct a required case management. Present for the inspection were Assistant Kristen Garcia, an additional assistant, and 7 preschool age children and one infant. The Licensee came down about 20 minutes into the inspection.
At approximately 9:15am, the LPA found that the 2nd assistant did not have a cleared background. A Type A citation with a Civil Penalty is being issued for this deficiency.
The LPA reviewed staff files, and found three of five were missing a Mandated Reporter certificate. LPA also reviewed the children files and found none to have the LIC9224 for the Non-Compliance conference held on May 11, 2023. Two Type B citations were issued for the deficiencies.
The following deficiencies were cited during today's visit:
1) Type A - Criminal Record Clearance
2) Type B - Mandated Reporter Certificate
3) Type B - Failure to maintain signed LIC9224s in the children files.
As a courtesy the LPA printed an LIC126 (Entrance Checklist for Family Child Care Homes, and two LIC9224s (Acknowledgment of Receipt of Licensing Reports). One LIC9224 is for the Non-Compliance Conference held on May 11, 2023, and the second LIC9224 is for the Type A deficiency issued on June 7, 2023.
The LIC9224s must be signed by currently enrolled children, and for newly enrolled children for the next 12 months. The LIC9224s must be kept in the children's files.
The LIC9224s, the LIC126, and the report was reviewed with Licensee Dorothea Raymond.
Appeal Rights and a Notice of Site Visit were given to the Licensee.
The Notice of Site Visit and reports must remain posted for 30 days. |
