On 7/13/26 at 3:15pm, LPA Briana Plumboy met with former Licensee's daughter Gloria Campos for a complaint investigation regarding the above allegation.

Based on the investigation by Community Care Licensing Department (CCLD) Investigation Bureau (IB), there was no preponderance of evidence to prove the alleged violation occurred. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is UNSUBSTANTIATED.

An exit interview was conducted with former licensee's daughter Gloria Campos.
A notice of site visit was given. Appeal rights provided and discussed.
