13 | On August 25, 2026, at 8:40AM, Licensing Program Analyst (LPA) Randy Miranda met with Licensee Zahaira Pereda Lopez to deliver the findings from a complaint investigation for the above allegation. LPA toured the facility and obtained a copy of the facility roster. Present for the inspection was the licensee, licensee’s assistant, licensee's adult daughter, and 7 children in care (3 infants, 3 toddlers, and 1 preschool age).
Based on information obtained through interviews, record reviews, and observations, the preponderance of evidence standard has been met. A child in care left the premises and wandered onto the sidewalk. A neighbor observed the child walking alone, secured the child, and remained with them until authorized adults or the guardian were located and identified. The allegation that the licensee allowed the child to leave the facility without supervision was SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 3, Article 06, Section 102417(a) is cited on the attached LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided.
Exit interview conducted and report was reviewed with Licensee, Zahaira Pereda Lopez.
