13 | On 9/18/2026, Licensing Program Analyst (LPA) Sha Fee Vang conducted an unannounced inspection to conclude the complaint investigation. LPA met with Director Tasha Avalos to discuss the purpose of the inspection and to deliver findings regarding the above allegation. A tour of the facility was conducted, and census was taken.
During the investigation, LPA Vang conducted interviews, facility observations, reviewed records, and obtained pertinent information relevant to the investigation. Observations and interviews disclosed that on several days, the toddler classroom operated out of ratio.
Based on observations, LPAs observed that during water play, staff were out of ratio with the number of children who remained outside while staff were transitioning back into the classroom, to prepare for the next activity. Based on observations, LPA observed that the toddler classroom was out of capacity at approximately 9:25 AM with 25 children in care with 6 staff members. Furthermore, observations disclosed that the toddler classroom continued to operate out of ratio.
