25 | On October 2, 2026, Licensing Program Analyst (LPA) Valentin Hernandez conducted an unannounced Case Management inspection regarding an Unusual Incident Report. LPA met with Director Lea Ann Gause. LPA toured the facility and census was taken.
LPA explained to the Director the purpose of the inspection was to follow up on a self-reported Unusual Incident that occurred on September 30, 2026, and was reported to CCLD that same day. During outdoor play, Child 1 (C#1) became physically aggressive with staff, repeatedly kicking, hitting, and attempting to run away, which required multiple staff members to intervene. C#1’s mother was contacted but stated she could not pick him up, and arrangements were made for the grandfather to pick up C #1, grandfather also struggled to escort the child home due to continued attempts to run off; C#1 ultimately left with the grandfather.
LPA inspected the playground and facility, and based on interviews and observations, there was no indication of a lack of supervision. The information gathered indicates this was an isolated incident, and staff responded appropriately and followed all required reporting procedures.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency cited during today’s visit. Exit interview conducted with Director Lea Ann Gause and a Notice of site visit will be posted for 30 days. Appeal Rights were provided and discussed.
