In course of this investigation records obtained by the Department’s Investigation Branch from DMV files indicate S1 resided at the licensed facility at least from June 2014 to July 2026 without a criminal record clearance.
Interviews conducted with parents of children enrolled revealed S1 has been present at the facility when children are in care. Neighbors interviewed described S1 as licensee’s spouse and know S1 as a resident of the licensed facility.
Licensee admitted that S1 has stayed over some nights and some weekends but denied his residency. Licensee provided a declaration that she is not married to S1 but has children in common.
Based on information gathered, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Title 22 section 102370 Criminal Record Clearance is being cited on the attached LIC9099D. A civil penalty is being assessed in the amount of $500.00.
LPA Del Mundo informed licensee, Cassondra Vermillion, that this report dated September 2, 2026, documents 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA informed licensee to provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
LPA provided licensee a copy of a blank LIC 9224 to be signed by parents/authorized representatives.
Exit interview conducted, and report was reviewed with the Licensee. A notice of site visit was given to Licensee, and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. |
