25 | On August 6, 2026, Licensing Program Analysts (LPAs) Aurelio Mendoza and Denisia Jimenez conducted an unannounced Case Management – Plan of Correction (POC) follow-up inspection to verify completion of the Plan of Correction for a deficiency cited during the Case Management – Incident inspection conducted on July 22, 2026.
LPAs met with Site Supervisor Darnisha Maiden, explained the purpose of the inspection, conducted a census, and toured the facility. Director Erin Wilcox was temporarily out of the office but participated by telephone throughout the inspection. LPAs reviewed the purpose of the visit and read this report aloud while Director Wilcox was on speakerphone. Site Supervisor Darnisha Maiden was present during the inspection and signed the report.
LPAs verified that the Plan of Correction was completed. Records confirmed staff received Personal Rights training on July 16, 2026, and that signed training acknowledgments were maintained for all applicable staff. LPAs also verified a signed protocol, dated July 23, 2026, outlining procedures to safeguard children's personal rights, including routine unannounced observations of staff by the Program Director. LPAs confirmed the required Plan of Correction documentation was submitted to LPA Mendoza by email on July 23, 2026, within the required timeframe, and verified that all supporting documentation was maintained onsite. Director Wilcox confirmed she continues to conduct routine walkthroughs to monitor staff practices and ensure compliance with Title 22 requirements.
Based on records reviewed and observations made, LPAs verified that the Plan of Correction has been completed and cleared.
An exit interview was conducted with Site Supervisor Darnisha Maiden while Director Erin Wilcox participated by speakerphone. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited during today's inspection. This report shall be made available to the public upon request. An LIC 9213, Notice of Site Visit, was issued and shall remain posted for 30 days.
