On 7/1/2022 at approximately 9:12am Licensing Program Analyst Sabina Dodoo met with Site Supervisor Essie Day for an Unannouced Complaint Inspection at Creative World Preschool. Present for the inspection were 17 children and 7 staff present. LPA explained the nature of the visit. LPA toured the facility.

The allegation was that the facility staff left child unsupervised during pick up hours. LPA interviewed 10 parents to ask of how satisfied are they with the supervision that is being conducted by the staff. At least 4 parents mentioned they were not satisfied. A parent mentioned a child once walked out of the door following the parent after the parent had picked up the child. Another parent mentioned that when picking up his or her child she or he felt that the child was unkept. The child was dirty and the face of the child was not clean.
Based on the interviews the preponderence of evidence standard has been met and the allegation of children being left unsupervised is found to be SUBSTANTIATED. A Type B citation will be given to the facility. The California Code of Regulations Title 22 Division 12 Chapter 1 # 101229(a)(1) Responsibility for Providing Care and Supervision is being violated. The Licensee acknowledge, that for Type B Deficiency, the licensee has to hold a meeting with the staff explaining licensing regulations for supervision. The supervision video titled "Supervising Children in Child Care Centers" must be watched. The Director can obtain the video at ccld.childcarevideos.org.

Each staff member attending the meeting must sign on an attendance roster. A copy of a meeting agenda and the signatures of each staff must be emailed to LPA at [email protected] no later than July 15th 2022.
An exit interview was conducted. Appeal rights were given and explained to the Site Supervisor Essie Day Notice of Site Visit was posted.

Based on LPA's interviews with 10 parents, at least 4 parents mentioned the staff was not providing adequate supervision to children in care. This poses a potential risk to Health, Safety and Personal Rights to children in care.
