25 | On September 29, 2026, Licensing Program Analyst (LPA) Lady Cabrera conducted an unannounced case management – other inspection. LPA met with Director Claudia Barriga. LPA toured the facility and a census was taken. LPA explained that the purpose of today’s inspection was to follow up on an exemption denial.
On September 27, 2026, a notice from the California Department of Social Services (Department), Care Provider Management Branch (CPMB), was generated and mailed to inform the facility that a criminal record exemption was denied for Adult #1 (A1).
During today’s visit, LPA explained to Director that a denied exemption means A1 may not work or be present in any agency licensed by the Department. LPA also explained that A1 is prohibited from having contact with clients, potential clients, or confidential client information of any agency licensed by the Department.
LPA confirmed with the Director that A1 had been employed at the facility; however, A1 no longer worked at the facility as of 07/15/2024, and the facility disassociated A1 from its roster on that same date. On today’s date, the Director completed the Confirmation of Removal form.
Verification of removal is complete.
An exit interview was conducted, and the report was reviewed with Director Claudia Barriga. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies were cited. This report shall be made available to the public upon request. LIC 9213, Notice of Site Visit, was provided and is required to be posted for 30 days.
