32 | Allegation- Staff does not ensure reporting requirements are followed. According to the RP and records obtained, C1 sustained an injury that was not reported until the following day. LPA spoke with RP who revealed that C1 sustained an injury above the neck that occurred on 07/27/26; however, it was not reported until 07/28/26 during pick-up. According to the RP, the facility policy for injuries above the neck requires a phone call and an ouch report on the same day the injury occurred.
RP also disclosed that staff did not report prior injuries involving C1 until the following day or several days later that occurred on 11/07/25, 03/02/26, and 06/05/26. LPA reviewed three ouch reports and observed that on 11/07/25 C1 was scratched on the left side of their nose and parents were provided the ouch report on 11/12/25. Ouch report dated 03/02/26 revealed that C1 scrapped their right knee and parents were provided the ouch report on 03/03/26. Ouch report dated 06/05/26 revealed that C1 was scratched on the left side of the cheek and parents were provided an ouch report on 06/09/26. LPA observed that the ouch reports were received after the injuries occurred because parents of C1 signed and dated each report.
LPA conducted interviews with S1 thru 6. According to S1 and S2, everything is reported to parents or guardians even if there are no visible marks. Per S1, incidents/injuries are reported in person, and an ouch report is provided at the end of the day unless the parent/guardian doesn’t want to wait for the written report they can be picked up the following day.
Staff interviewed revealed that parents/guardians are notified of incidents/injuries via PROCARE (online communication app for parents), ouch/incident reports, or phone calls. According to staff, head injuries require a phone call that is done by administration and not teachers. Staff also corroborated that incidents/injuries should be reported at the end of the day during pick-up.
According to S1, parent of C1 reached out regarding the head injury that occurred on 07/27/26 and disclosed that they did not receive a phone call. S1 stated that S2 signed the ouch report and because it was a head injury the facility should have called the parent. S1 stated that there is a possibility that staff reported the injuries involving C1 to their parent or guardian during pick up; however, it cannot be confirmed or denied since the staff who signed the ouch reports are either not present, may not remember, or no longer working at the facility. S1 also revealed that there has been occasions when staff, parents, or guardians have written the wrong date on the ouch reports that may lead to a misunderstanding.
