On 9/15/22, Licensing Program Analyst (LPA) Steven Tung conducted an unannounced Case Management visit regarding the current status of the day care facility. Due to COVID-19 precautionary measures were taken, licensing staff present during inspection wore appropriate personal protective equipment. LPA met with Licensee Anna Tang who guided LPA on a tour of the facility. Licensee's spouse was also present during the visit.

The purpose of the Case Management visit was to determine if the day care facility has reopened as the last request by licensee for inactive status expired on 3/31/2018. The Licensee has requested to remain on inactive status through 9/15/2023.

LPA obtained an updated phone number and provided Licensee with licensing documentation regarding the re-opening of the day care facility.

Exit interview conducted and report was reviewed with Licensee Anna Tang.
