On October 28, 2026, at approximately 10:11 a.m., Licensing Program Analyst (LPA) DeCondia Ferguson conducted an unannounced follow-up visit at UCLA Early Care and Education Krieger Center to conclude the investigation regarding an Unusual Incident Report (UIR) involving a Tylenol ER pill discovered in the infant outdoor play yard on 09/30/2026.

Upon arrival, LPA met with Director Arpine Panosyan, to whom the purpose of the inspection was announced. LPA conducted a tour of the facility, during which a census was obtained. LPA observed the following:

Bunnies: 7 children and 5 staff; Pandas: 12 children and 5 staff; and Penguins: 10 children and 5 staff

A total of 29 children and 15 staff were present in the three infant classrooms.

According to the UIR, on 09/30/2026, a Tylenol ER pill was discovered on the ground in the infant outdoor play yard while children from the Pandas and Penguins classrooms were utilizing the area.

During the investigation, LPA interviewed Staff 1 (S1), Staff (S2), and Director (D1).

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

S1 stated that she discovered a white, oblong-shaped pill in a grassy area bordering the maze bush while supervising children from the Pandas classroom. S1 reported that no children were in the immediate area where the medication was discovered and that she did not observe any child touching, tasting, or ingesting the medication. S1 retrieved the pill and notified S2.

S2 confirmed that S1 informed her of the discovery and provided her with the medication. S2 stated that she asked staff whether the medication belonged to them and that staff denied ownership. S2 subsequently notified D1.

D1 stated that upon notification, she conducted an additional inspection of the outdoor area and did not observe any additional medication or potential hazards. D1 reported that staff and interns were questioned regarding ownership of the medication and denied that it belonged to them.

D1 further reported that the medication appeared intact, with no visible bite marks, moisture, or broken pieces. D1 notified parents of the incident and requested that they report any symptoms or concerns. According to D1, no parents reported symptoms or concerns, and no children displayed unusual behavior following the incident.

During the investigation, S2 initially reported observing Facilities personnel walking through the infant outdoor area. D1 stated that Facilities personnel had been working in a separate area outside the gated infant yard.

During the follow-up visit, LPA and D1 revisited the outdoor area to clarify the location where Facilities personnel had been working and their potential access to the infant yard. Based on the additional visual assessment, it was determined that S2 may have confused the maintenance work area with the infant yard where the medication was discovered.

D1 reviewed the facility's sign-in records for 09/30/2026 and reported that no documentation identifying Facilities personnel was available. D1 explained that Facilities personnel typically enter through the front entrance but may have accessed the maintenance area through the rear entrance on the incident date.

Neither LPA nor D1 interviewed Facilities personnel, as the individuals present could not be identified.

Regarding outdoor safety inspections, D1 stated that staff are required to complete a daily environmental safety inspection before children enter the outdoor play area. D1 explained that the facility utilizes a laminated safety checklist that is erased following each inspection. Therefore, a completed checklist for 09/30/2026 was unavailable for review. Although S2 reported that an inspection had been conducted, the individual responsible for completing the inspection could not be identified.

LPA provided technical assistance to Director Arpine Panosyan regarding the importance of conducting and documenting daily outdoor safety inspections prior to children entering the play area. LPA recommended maintaining inspection records for future reference and ensuring outdoor areas remain free of potential hazards.

Based on interviews, observations, and information obtained during the investigation, no child was reported to have touched or ingested the medication, and no injuries or symptoms were reported. The facility took action to remove the medication, inspect the outdoor area, notify parents, and remind staff of the importance of conducting environmental safety inspections.

The source of the Tylenol ER pill could not be determined. No evidence was obtained establishing that the medication belonged to facility staff or Facilities personnel.

The investigation is concluded. No deficiencies are being cited at this time based on the information obtained during the investigation.

An exit interview was conducted with Director Arpine Panosyan, and a copy of this report was provided to the facility representative.

A notice of site was provided and must be posted for 30 days. Appeal Rights provided.
