On 7/31/2026 at 9:00 AM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Complaint Inspection for the purpose of delivering findings for the above allegation. LPA announced purpose of inspection and was allowed entry to facility by facility representative Vivian Beans. LPA took a self guided tour of the facility. There were 51 children present with 10 staff upon arrival.

During the investigation LPA obtained a copy of the facility roster, a copy of the employee roster and reviewed injury reports, attendance sheets, staff files and conducted interviews with staff, and parents.

Information gathered confirms that the facility is not meeting regulation that children shall not be left without the supervision of a teacher at any time. Interviews with Staff 1, Staff 2 and Staff 5 confirmed that unqualified facility staff are left with children for supervision purposes when the facility is short staffed particularly at morning transition or for breaks.

The incident was witnessed by Staff 1 and Staff 6. Staff 6 was with the child at the time of the incident. Staff 1 and Staff 6 stated that the child was sitting at a table and was throwing themselves onto the floor repeatedly during lunch transition. Staff 6 allowed the child to calm down while observing him and providing guidance and assistance to stand and sit to prevent injury. Scratches appear to be surface level with broken skin and redness on the surrounding edges. The size of scratches could not be calculated but appear small in relation to the facial area. There was a small dime size red bump on the inner edge of Child 1’s left eyebrow and a darkening of the skin on his chin. Injuries although varied were minimal and self inflicted. Staff were present during the incident and observed the incident take place. Staff took measures to minimize the damage by providing assistance and guidance.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted with Facility Representative, Principal Vivian Beans.
