On Monday, October 26, 2026 at 12:40 p.m., Licensing Program Analyst (LPA) C. Nguyen conducted an unannounced annual inspection and met with Director Susan Mukai, who guided LPA on a tour of the facility. The preschool is located inside Los Angeles Hompa Hongwanji Buddhist Temple. This program operates Monday – Friday from 7AM to 6PM and serves children ages 2 ½ until entry into 1st Grade. All areas identified on the facility sketch were toured and inspected both indoors and outdoors. Per the Director, there are 30 children enrolled. Teacher and child ratio were observed, and present staff are fingerprint-cleared: 19 children and 5 staff. The following classrooms were inspected: Rainbow Classroom: 12 children with 2 staff; Starlight Classroom: 7 children with 2 staff. Per Director, Sunshine Classroom is not in use due to low enrollment, but is used for napping time and was inspected for health and safety.

Sign in and out takes place at the main entrance area and were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. The Director stated that no firearms, weapons, poisons or bodies of water are located on the premises, and LPA did not observe any of the above-mentioned.

The following documents were posted in a prominent, publicly accessible area at the facility: Facility License, Notification of Parents' Rights (PUB 393), Personal Rights (LIC 613A), Menu Snack (weekly and past 30 days are available upon request), Earthquake Preparedness (LIC 9148), Child Passenger Safety Poster (PUB 269), and Daily Schedule.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA entered the classrooms and observed furniture, manipulatives, toys, games, and books made accessible to the children to be age appropriate and free of sharp, loose, or pointed parts. LPA observed each classroom has a cubby for children to store their personal belongings. The facility provides filtered water to children in care. Children bring their own water bottles and bottles get refilled as needed. Napping equipment in the form of cots were observed and Director stated that the linen and cots are washed every Friday. Linen are brought and washed by the parents. Licensing staff observed step trash bins with a tight-fitting lid inside the classrooms. LPA observed the girls (4 toilets/3 sinks) and boys (2 toilets/4 urinals/3sinks) restrooms in safe and sanitary operating conditions, having working toilets, sinks, hand soap dispensers, and paper towels. There is also a diaper changing station in each restroom. During time of inspection, LPA observed cleaning solutions stored in classroom cabinets with child-safety latches, making it inaccessible for children to open. There is a custodian room next to the boy’s restroom where more cleaning supplies are stored and inaccessible to children. Director stated that they currently have (2) two children on medication or severe food allergies. LPA observed the ill isolation area to take place in the Lotus Room (staff room) and a cot is available if needed.

Heating, lighting, and ventilation were evaluated by LPA. LPA observed central AC/heater and vents located on the ceiling. LPA observed the thermostats located in the classrooms and temperatures to be between 70-71 degrees. LPA observed the fire extinguishers in the classroom and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 10/21/2026. Smoke detectors were observed to be hard-wired to the system and not were not tested on this visit. Carbon monoxide detectors were not observed. The last emergency drill was conducted on 10/26/2026. First aid kit supplies are kept inside the classrooms and LPA observed the kit to be fully equipped.

LPA observed two outdoor playground yards (Lower and Upper yard) to be fully fenced. The outdoor play equipment was observed to be free of sharp, lose or pointed parts. Outdoor activity space surface is maintained in a safe condition and is free of hazards. Areas around and/or under climbing equipment, swings, slides and similar equipment have cushioning material to absorb a fall. Drinking water is readily available outdoors. There is adequate shade in the play yard via plants and tarps/sails. LPA advised Director and staff to do a quick walkthrough before children play outside.

Per Director, the facility provides (3) three snacks and children bring their own lunch to the facility that are placed in their cubbies and labeled with the child’s name. PAGE 2 OF 4

LPA informed licensee that any food brought from the children's homes, shall have the container labeled with the child's name and properly stored or refrigerated. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage.

LPA reviewed the children’s roster, children’s files, staff files, and sign in/out sheets. LPA provided consultation on staff and children files. LPA observed Director’s American Heart Association Pediatric First Aid/CPR certification and expires 11/2027 and has proof of immunization against Pertussis, MMR, and Influenza declination. Director has completed the mandated reporter (AB 1207) training and expires 2/23/2028. Director was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com.

All individuals subject to criminal background review have obtained a criminal record clearance. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed on 10/2023 – 8/2024 in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA also informed Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The facility provides Incidental Medical Services-IMS. LPA reviewed storage of medication and (equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

At this time, the licensee is not in compliance with California Title 22 Regulations. The deficiency listed on the following page was observed by LPA and is being cited in accordance with California Code of Regulations Title 22 or Health and Safety code. One type B deficiency is being issued today for regulation. Please see attached LIC 809-D for citation.

The Notice of Site Visit (LIC 9213) – A notice of site visit was given and must remain posted for 30 days. Appeal rights were given. Exit interview conducted and report was reviewed with the Director Susan Mukai.
