On October 13, 2026, at approximately 09:11 a.m., Licensing Program Analyst (LPA) DeCondia Ferguson conducted an unannounced Case Management – Incident inspection at Canyon Crest Preschool regarding an Unusual Incident Report (UIR) received by the Department on September 11, 2026. The UIR concerned an incident that occurred on September 11, 2026, at approximately 9:55 a.m., involving a four-year-old child (C1) who reportedly tripped and fell while running on the outdoor play structure, sustaining a laceration near his eyebrow.

Upon arrival, LPA met with Director Pauline McPeake, to whom the purpose of the inspection was announced. The Director guided LPA on a tour of the facility, during which a census was obtained. LPA observed 71 children present. Per Director, 83 children are currently enrolled.

During the facility tour, LPA observed the following classroom census and staffing:

In Room Ravens, LPA observed 15 children with 3 staff; Room Goldfinch, 13 children with 3 staff; Room Starlings, 14 children with 3 staff; Room Robins Full Day 1, 11 children with 3 staff; Room Robins Full Day 2, 11 children with 3 staff; and Room Robins Part Day, 7 children with 3 staff.

During today's inspection, LPA conducted interviews with C1, five staff members (S1–S5), and C1's parent (P1) to obtain information regarding the circumstances surrounding the incident, supervision, staff response, and medical treatment.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During the interview, C1 stated that he was chasing another child on the outdoor play structure when he tripped on the wooden steps and struck his head against the wood. C1 identified S1 as the teacher who assisted him following the incident and stated that an ice pack was applied to his injury. C1 did not report any additional injuries.

Staff interviews revealed that no staff member directly witnessed C1 fall. S5 reported being positioned near the play structure when C1's crying alerted S5 to the incident. S5 observed C1 holding his head, assisted him, and transferred him to S1 for first aid. S1 stated that upon receiving C1, S1 observed bleeding from the injury and immediately applied gauze, pressure, and an ice pack. S2 notified administration, and the Director subsequently assisted with C1's care.

During interviews, staff estimated that approximately 27 to 30 children were present on the playground at the time of the incident. Per Director, the facility's established playground supervision practices include positioning staff throughout the outdoor play areas, including near the play structure, grassy area, sandbox, and pergola, with an additional staff member circulating throughout the playground. Director stated that required staff-to-child ratios were maintained at the time of the incident. Staff interviews did not identify concerns regarding inadequate supervision contributing to C1's injury.

During the inspection, LPA conducted a physical inspection of the outdoor play structure, including the wooden steps and surrounding play areas where C1 reportedly fell. LPA observed the steps and surrounding structure to be free of observable hazards, including damaged or protruding materials, debris, or other visible conditions that could pose a tripping or injury hazard. LPA photographed the play structure to document the condition of the area at the time of the inspection.

P1 confirmed receiving notification from the facility regarding C1's injury and stated that upon arrival, C1 was calm and received appropriate care from the Director. P1 transported C1 to his pediatrician, where the laceration was treated with skin adhesive and Steri-Strips. Per P1, C1 was not diagnosed with a concussion or other head injury and did not require additional medical restrictions. P1 reported that C1 has recovered well and expressed no concerns regarding the facility's supervision or response to the incident.

During the inspection, LPA obtained copies of the staff roster, children's roster, and children's sign-in sheet dated September 11, 2026. LPA also obtained a photograph of C1's injury and text message correspondence between P1 and the Director regarding the incident and subsequent medical treatment.

Per Director, following the incident, staff discussed playground safety expectations with the children, including using walking feet while on the play structure and engaging in running activities in the designated grassy area.

Based on interviews conducted, observations made, and documentation reviewed during the investigation, the incident appears to have resulted from C1 accidentally tripping while running on the outdoor play structure. Although no staff member directly witnessed the fall, information obtained from C1, staff, and P1 was consistent regarding the circumstances surrounding the incident. LPA's physical inspection of the wooden steps and surrounding play structure did not identify observable hazards that may have contributed to the injury.

Information obtained during the investigation did not establish that inadequate supervision contributed to C1's injury. Staff responded promptly, provided first aid, notified C1's parents, and ensured C1 received appropriate medical attention. P1 confirmed that C1 received medical treatment and expressed no concerns regarding the facility's supervision or response to the incident.

Based on observations made, interviews conducted, and records reviewed, the investigation did not establish a violation of applicable Title 22 regulations. Therefore, no deficiencies are being cited during today's inspection.

An exit interview was conducted with Director Pauline McPeake, during which the investigation findings were discussed and the report was reviewed. A copy of this report and the Notice of Site Visit (LIC 9213) were provided to the Director. The Notice of Site Visit shall be posted in a prominent location for 30 days. Appeal Rights were provided.
