According to staff interviewed during the course of the investigation, S1 admitted to grabbing C1 firmly by the arms for a diaper change. S1 stated that a scratch occurred on C1’s arm when S1 grabbed the child. S1 stated that S1 contacted C1’s parent and informed the parent that S1 had accidentally scratched C1 while attempting to change the child’s diaper. S1 also stated that photographs of the scratch were provided to the parent and that the parent signed an Unusual Incident Report.
LPA Gil interviewed three additional staff members who were present in the classroom at the time of the incident or during the transition period surrounding the incident. None of the staff interviewed witnessed the incident. However, the staff stated that they observed a scratch with redness on C1. All parents interviewed stated that they are notified by telephone and provided an incident report when their child is injured while in care. The parents also stated that they have not experienced any unexplained injuries involving their children while in care.
