The Licensee used her cell phone to begin playing the footage. LPA stood next to Licensee and Staff 2, as footage starting at 11:50 a.m. was being reviewed. In the footage, LPA observed S2 enter the camera frame carrying a child wearing a yellow shirt and stating “This woman said…” The Licensee then stopped the video and told LPA she would play the video from the computer instead. LPA observed the Licensee use a laptop for a few minutes. LPA informed Licensee that the footage could be viewed on the cellphone. License returned to the LPA and attempted to play the footage again. However, the part of the recording of S2 walking in with a child was no longer available. The segment showing S2 carrying the child had been deleted.
During an interview, Adult 1 (A1) disclosed that on 06/30/2026 at approximately 12:10 p.m., A1 was driving down the street when A1 observed a child wandering onto the street unattended. A1 picked up the child and walked toward the facility, where S2 was standing by the black door and took the child from A1.
Based on A1’s interview and the dashcam footage obtained from the A1’s vehicle which captures the minor child wandering onto the public street unsupervised, the allegation that the Licensee did not ensure proper supervision resulting in a child leaving the facility is Substantiated. Type A citation issued, please see LIC 9099D.
LPA Laureano informed Licensee Keyla Bell that this report dated 10/12/2026 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Laureano informed Licensee, Keyla Bell, to provide a copy of this licensing report dated 10/12/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
