**This is an amended report to dismiss a unsubstaniated finding, with a Substanaited finding, from the report that was issued on 10/06/2026. **

On 10/06/2026 Licensing Program Analyst, LPA, Tyra Chavies, conducted an unannounced case management visit at Minter FCCH. LPA met with licensee, Catherine Minter and toured the facility indoors and outdoors. There were 1 child being cared and supervised by licensee.

08/13/2026 LPA Chavies observed daily facility routine and observed children in care

07/22/2026 LPA Chavies observed children in care, received personnel and facility roster, conducted in person interviews with Licensee, S1, P1, P2, C1, C2 and C3

“Based on LPAs observation, interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Per the California Code of Regulation. (Title 22, Division 12 & Chapter number 1) this facility is being cited a Type B. Please LIC 809-D
