complaint inspection on June 23,2026, LPA observed gallons of milk that had expiration dates of May 24, 2026, and May 26, 2026. The director admitted that gallons of milk were frozen prior to the expiration date.
A second allegation was received alleging that staff do not properly maintain the outdoor play area. Based on observations during the initial visit, it was revealed that one of the play equipment (dramatic kitchen toy) in the outdoor play area had a broken window that has sharp edges. The director was notified regarding the broken window, and the window was detached from the play equipment. The window was later thrown into the trash bin.
Based on the information obtained, the preponderance of evidence standard has been met. It was determined that the allegations: staff did not provide adequate food service for daycare children and staff do not properly maintain the outdoor play area were substantiated.
Type B deficiencies were cited in accordance with the California Code of Regulations Title 22, Division 12, Chapter 1, regulations 101227 Food Services and 101239 Fixtures, Furniture, Equipment and Supplies. See attached LIC 9099D.
Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted, and a copy of this report and appeal rights were reviewed with facility director, Edrenia Williams. |
