During the initial visit to the facility, the LPA and the director went inside the kitchen and checked the backdoor of the kitchen. LPA was able to open the backdoor from the inside using the metal handlebar. It was also revealed that the kitchen backdoor was designed that way to comply with the Fire Department. LPA was informed that the kitchen is an off-limit area and a kiddy gate is used as a barrier.
A second allegation was received, alleging that staff did not ensure that daycare children drank from a non-contaminated water source. During the initial visit, LPA went to the cafeteria and checked the water source. LPA observed the water source used a filter system. Also, during the investigation the facility water lead test revealed the facility did not exceed the allowable amount of lead in the water.
A third allegation was received alleging that staff are commingling daycare children. LPA reviewed the Sign In and Out Sheet of the children, staff files, and the timecard of the staff for the months of April and May 2026. Based on records review, it could not be determined if the center is commingling infant, preschool and school age children at the end of the day after 5:00 PM. Confidential interviews revealed that the children stay in their respective classrooms or in the cafeteria with the staff providing care and supervision. It was also determined during the interview that school age children play with other children of the same age group. Also, during the LPA’s initial visit at the facility, LPA observed did not observe preschool or school age children in the infant classroom.
Based on record review, confidential interviews, and LPA observations, there is not enough evidence regarding the allegations, staff did not ensure the facility door was operating properly, staff did not ensure that daycare children drank from a non- contaminated water source, and staff are commingling daycare children.
Therefore, the allegations are deemed unsubstantiated at this time. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur.
Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted, and a copy of this report and appeal rights were reviewed with facility director Edrenia Williams. |
