On February 14, 2025, Licensing Program Analyst (LPA) Annelise Villa conducted an unannounced complaint visit to deliver the findings. Due to facility relocation, the complaint visit was conducted at Licensee's current facility (197700870). LPA was met by the Licensee Stephanie Armstrong who permitted entry to the facility. LPA toured the facility with the Director according to the facility sketch. Upon arrival, LPA observed 6 preschool age children and 1 infant with Licensee and 1 staff caring for them.

During the investigation, LPA collected pertinent documents, completed record reviews, and conducted confidential interviews. Complaint allegations state Licensee transported children from school to a family child care home, instead of the child care center they were enrolled in. During interviews conducted, Licensee confirmed transportation was provided to the children to an associated family child care home. LPA verified the facility did not have any documented agreements for transporting children to their associated facility at that time. This is a Type B citation, see the LIC 9099D for the details.
