Licensing Program Analyst (LPA) Denise Miranda conducted an unannounced complaint inspection to the above facility on 11/16/2022. LPA arrived to the facility at 7:35AM and met with Yolanda Allen, Licensee, who guided analyst on a tour of the facility. There were no children present upon arrival. At 7:45AM LPA observed one child arrived.
During the investigation LPA interviewed, interviewed parents, obtained a copy of the children's roster, and obtained copies of supporting documentation.
Information provided by the reporting party indicates that day care is dirty, Licensee restrained day care child in stroller, and Licensee did not feed day care child.
Per During the course of the investigation, LPA did not observe the facility to be dirty. Per Licensee, a mother of one child brought a stroller so that Licensee can utilize for walking during outdoor time. Licensee also stated that they she is are participating in a food program in which the requirement is to provide food to all children in care.

Parents #2, #3 and #4 disclosed that Licensee does not restrain day care children in a stroller. Parents also disclosed that the Licensee is feeding the children.
Children were not interviewed due to their age.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Yolanda Allen, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
