On 7/17/2024 Licensing Program Analyst (LPA) Judy Laureano arrived at above mentioned facility unannounced, to conduct a Case Management – Other inspection. LPA Laureano was greeted by Maria Valdez. LPA observed 6 children in care.

Licensee Dmitri Kondrashov was not present when LPA arrived at 10:55 a.m. LPA contacted Licensee and left a message.

M.Valdez stated that Licensee is currently out of state and she not certain when Licensee will be back.

On deficiency was cited during today’s visit in accordance to the California Code of Regulations Title 22, Division 12, Chapter 1.



Upon on receipt of this report, the Licensee shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.

An exit interview was conducted, and report was reviewed with Maria Valdez

LPA is requesting Licensee to submitt a declaration- LIC855 with a written plan to LPA when leaving out of the state and future plans of the program. In the plan Licensee will establish whether Licensee will be permanently closing and/or Licensee's planned time at FCCH

Based on observation and interview, the licensee did not comply with the section cited above, Licensee is out of the state and M. Valdez can't confirm a date of return, which poses/posed a potential health, safety or personal rights risk to persons in care.
