On 06/24/2026, Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced visit to the above-mentioned facility for the purpose of delivering complaint findings. Upon arrival, LPA was greeted by Staff Member 4 (S4) and directed to Staff Member 3 (S3). LPA introduced herself to S3 and explained the purpose of the visit.

The investigation regarding the above-referenced allegation was conducted by LPA Castellanos.

During today’s walkthrough of the facility, LPA observed twenty eight (28) children in care with four (4) staff members providing supervision. Shortly thereafter, at approximately 11:26 a.m., Assistant Director Anna Sutherland arrived at the facility and met with LPA.

On 04/07/2026 Licensing Program Analysts (LPA) Cristina Castellanos and Judy Laureano conducted the initial complaint investigation at the above-mentioned facility. The premises also house the infant program (Lic. #197494670) and Rising Stars Academy Private School. During the investigation, LPA requested and reviewed the following documents: Children’s Roster, Staff Roster, Parent Handbook, Incident Reports for March 2026, and an updated Facility Sketch. LPAs also conducted staff and children’s interviews.

On 06/24/2026, LPA concluded interviews with all relevant parties.

Based on observations, interviews, and record review, no information was obtained indicating that staff do not treat children with respect. Although the allegation may have occurred or may be valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. Therefore, the allegation is deemed UNSUBSTANTIATED.

An exit interview was conducted with Assistant Director Anna Sutherland. A copy of this report and appeal rights were discussed and provided to the Assistant Director. A Notice of Site Visit was issued and must remain posted for 30 days. Failure to comply with posting requirements may result in an immediate civil penalty of $100.
