25 | An unannounced Case Management inspection was conducted by Licensing Program Analyst (LPA) Thelma Razo. LPA stated the reason for today's visit is to activate the license that was previously on inactive status per the Request for Inactive Child Care Status which expired on 10/312021. LPA Razo met with Licensee Marianna Montanaro who stated that she is remodelling her house and wanted to continue inactive status starting today, 2/23/2022 through 8/30/2022. A completed LIC 9211 form (Request for Inactive Child Care License Status) was submitted during this inspection. There are no children in care during inspection.
LPA reminded Licensee to submit an LIC9211 prior to re-opening. Licensee agreed to the following:
- that she will not provide child care
- will continue to promptly pay annual license fee
- will inform Licensing of any changes in the dates prior to re-opening by submitting a new LIC 9211
- will be incompliance with all licensing laws and regulations upon reopening to include but not limited to criminal record clearances for all adult staff and residents including children who turn 18 during inactive period, current pediatric CPR and first aid, functioning smoke alarms and current fire extinguisher
LPA reviewed with Ms. Montanaro LIC279 Application for a Family Child Care Home License and licensee has indicated that her daughter Alessandra Montanaro is no longer a co-licensee and has moved out of the facility and there are 2 adult family members currently living in this home and no minors.
Licensee is to submit an updated LIC279 and updated facility sketch by 3/15/2022.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with Licensee Marianna Montanaro.
