32 | Staff 9 stated that they observed Child 1 and Child 2 struggling over a toy. Staff 9 turned away after being called by another child, Child 2 let go of the toy and Child 1 fell backwards into a basket hitting his head with the plastic baby doll upon falling back. Staff 9 states that the child cried and was assessed for injury by Staff 9. No injury was observed and child did not identify where they were hurt. An ice pack was applied per child request, but no injury was seen at the time. Staff 7,9 and 10 state that the injury was seen at pick up and an injury report created for a raised bump on the head. Injury report was observed by LPA signed and dated by the parent.
Facility was observed to have a plan for injuries. Staff interviews confirmed that there is a procedure for injury. The procedure is staff that observed the injury assess the injury and notify the head teacher. Staff then write an injury report, and the report is then signed by the director and paper clipped to the sign in and out sheet for the parent. A copy is placed in the child’s file and the parent notified depending on the severity of the injury. LPA was unable to see pictures of the injury however complainants stated the first injury was detailed by the child as falling off of a scooter for which the injury was a dime sized bruise that was observed later in the evening hours after pick up. Due to the injury being observed later in the day it cannot be determined if the injury occurred at the facility or if the staff would have been able to create an injury report as the injury may have appeared after pick up. The second injury was observed by the reporting party at pick up. Reporting parties stated that an injury report was provided but only upon request. The injury was a raised bump on the forehead. LPA observed injury two which is a slight pink near the hair line with a slight bump. Child is seen in pictures content. Child 1 does have long hair which may have hampered identification of the injury. Injury appears to match the incident. Parents interviewed state there are no concerns regarding the facility and that they are notified if their child is injured by phone or with an injury report at pick up. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
