(RP), 3 Parents, Licensee and Assistant. There was no corroborating statements made during the interviews. Staff interviewed stated that they were not a witness to or had any knowledge of the above allegations.

Allegations: Licensee did not report injuries to a parent/ authorized representative.
The above allegation was found unsubstantiated due to information gathered during this investigation. It was reported by staff assistant Child #1 was injured at the facility and informed LPA’s child self conflicted injury when playing outside and injury was reported by Licensee to Child #1 guardian via written notice. LPA’s requested supportive document and document was not obtained and no proof was provided. There were conflicting information provided by staff assistant and reporting party.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and a copy of the report and appeal rights were provided to Licensee Columba Valdez.
