 THIS IS AN AMENDED REPORT
On June 25, 2026 at 8:45 a.m., Licensing Program Analyst (LPA) Peter Bishop arrived at the above facility to conduct an unannounced complaint inspection to conduct interviews and deliver findings. LPA announced purpose of inspection and was granted entry to facility by Facility Representative (FR) Vijayalaxmi Shadagopan. LPA met with (FR) Vijayalaxmi Shadagopan and toured the facility. At 10:30 a.m. LPA Bishop continued the visit with Director Cassandra Wijeyewickrema. There were 10 children present at the time of this visit. All center staff that were present during today’s inspection had fingerprint cleared and associated with the facility.

During the course of this investigation, interviews were conducted with the Reporting Party (RP), 3 Parents, 3 Teachers and 2 Children. There were no corroborating statements made during the interviews. Everyone interviewed stated that they were not a witness to or had any knowledge of the above allegations. LPA Bishop observed the communication between staff and children on all previous and current visits.

Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and a copy of the report and appeal rights were provided to the Director Cassandra Wijeyewickrema.
