Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced case management- other site inspection to ensure due to Inactive Status has expired and licensing fees have not been paid.

LPA met with Licensee, Trina Nunley. The licensee stated that there are no children enrolled, LPA observed no children present during inspection.

At the time of inspection, licensee requested that the license be permanently closed, as she is unable to pay the fees due.

LPA advised licensee that any outstanding fees will need to be paid, if she applied for a licensee in the future.
