32 | Per the Reporting Party it was alleged. “In the toddler room, a teacher assistant filled out the nap logs and would “fake other teacher’s assistant’s initials.” LPA Perry was unable to interview the reporting party; No information was provided.
LPA Perry interviewed parents. No disclosures were made pertaining to the above allegation.
LPA Perry interviewed children. No disclosures were made pertaining to the above allegation.
LPA Perry interviewed Staff #1-Staff #12, who all indicated that they have never observed anyone forge a sleep log or have forged it themselves.
During an interview conducted with the Director, the Director stated that she ensures staff continuously complete the required toddler nap checks. The Director explained that she is familiar with the staff assigned to the toddler classroom and, therefore, is aware of which staff initials should be reflected on the nap logs. The Director stated that cameras are located throughout the facility and can be viewed from her office, which allows her to monitor the classrooms and observe staff conducting nap checks. The Director further stated that staff are responsible for conducting the required checks and accurately documenting their own observations and initials on the nap logs.
During the inspection, LPA Perry observed toddler nap time checks and observed staff conducting nap checks and documenting the checks on the nap log form. LPA Perry observed that the initials documented on the nap log corresponded with the staff conducting the checks at the time of LPA’s observation. LPA did not observe staff completing or initialing a nap check on behalf of another staff member. LPA also reviewed available toddler nap records for weeks of August 3, 2026-August 7, 2026, August 10, 2026-Agust 14, 2026 and from August 17, 2026 – August 21, 2026. LPA Perry observed the records to be completed and the initials with staff assigned to the classroom. However, during the review of the toddler sleep logs LPA Perry observed uniform 15-minute check times for each toddler nap log. The forms also had the start and end times of their naps which did not always correlate with the uniform check times. Director acknowledged that staff had a misunderstanding on the nap logs. However, no evidence was obtained to indicate that sleep logs were being forged. However all times of toddler sleepings were documented and initialed.
Based on LPA Perrys observations and the records reviewed, LPA did not obtain evidence to corroborate that staff were falsifying another staff member’s initials on the toddler nap logs but did address the how they were being completed. Therefore, the allegation is determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated.
