An unannounced case management was conducted today by Licensing Program Analyst (LPA), T. Tran at the above licensed home. Upon arrival, LPA met with licensee's adult son, Elmer Sapiens and observed no children in care. LPA explained the purpose of today’s visit is to review and provide licensees a copy of the Decision and Order (D&O). Per Elmer Sapiens, licensees are currently away from the home but he feels comfortable receiving the D&O on the behalf of the licensees.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee's adult son, Elmer Sapiens.
