32 | Allegation 1: Staff do not ensure day care child's diapering needs are met.
The Reporting Party (RP) stated that Child #1 (C#1) arrives at approximately 9:30 a.m. and is reportedly not changed until lunchtime/nap time, around 12:00–12:30 p.m., resulting in C#1 frequently smelling of urine.
Director stated that children are checked hourly and are not permitted to remain in a diaper for more than three hours, even if the diaper is dry. Director stated that diaper changes are documented through daily reports. Upon arrival, children are greeted at the door and staff complete a wellness check. Director reported that parents receive a daily report, which documents diaper changes, including the staff member who completed the change and a signature at the bottom of the report. Director denied knowledge of any incidents in which a child remained in a soiled diaper or clothing longer than expected.
Staff #1-Staff #6 (S#1-S#6) reported that children are checked for diapering needs at least hourly, with some staff indicating that children are checked more frequently when needed. Staff reported that children are changed when wet or soiled and that children who remain in diapers are changed after three hours, even if the diaper is dry. S#1-S#6 described the use of diapering checklists and/or daily reports to document diaper checks and changes.
Parents interviewed stated that they do not have any concerns regarding the facility or staff and reported satisfaction with the care provided.
During the course of the investigation, LPA reviewed potty-training daily reports for four (4) preschool-aged children, including potty training report for C#1, for the following dates: 06/04/26, 06/11/26, 06/12/26, 06/18/26, and 06/24/26. The potty-training reports documented diapering observations completed by staff on an hourly basis and indicated whether the child was observed to be “Dry,” “Wet,” or had a “Bowel Movement.”
Based on interviews conducted, documentation reviewed, and LPAs observations, there was no indication or evidence to support the allegations that staff do not ensure day care child's diapering needs are met. Based upon the evidence as presented above, the allegation has been determined to be UNSUBSTANTIATED.
