32 | During staff interviews, no information was disclosed to support the above allegation. S #3 stated that the program maintains two coaches present at all times, regardless of the number of children in a group. All Staff reported using their walkie talkies to communicate with one another and always inform other staff members of their children count and when they need to step away from the group. S #2 stated they personally rotate throughout the activity area, moving from one location to another while monitoring the perimeter to ensure children remain within the same designated area. All staff consistently reported that they cannot recall any instance when a child was left alone or without visual supervision.
Parent interviews made no disclosures pertaining to the above allegations. All parents stated that they are very satisfied with the facility and reported that both they and their children enjoy all of the coaches. Parents further stated that their children have not expressed any concerns regarding the facility or its staff.
During interviews with children, C #3 and C #4 disclosed incidents in which they reported being without staff supervision. C #3 stated that they stopped to get a drink of water outdoors while the remainder of the group continued into the classroom, resulting in C #3 being left behind briefly until rejoining the group. C #4 stated that they entered a classroom alone, sat down, and read a book until the Program Manager came to locate them. These disclosures were inconsistent with staff interviews, during which staff stated they could not recall any instance in which a child had been left alone or without visual supervision.
Children interviews further disclosed concerns regarding staff supervision. C #1, C #2, and C #4 reported observing staff using their cell phones to place orders through Amazon, Starbucks or for food. C #3 reported that staff are sometimes with the children and, at other times, are off to the side doing other things. This disclosure corroborates the above allegation that staff do not provide adequate supervision.
Based on interviews, observations and disclosures made during the department investigation, the preponderance of evidence and the standard has been met, therefore, the allegations are found to be substantiated. California Code of Regulations (Title 22 Division & Chapter), are being cited on the attached deficiencies page LIC 9099-D. These incidents pose a risk to the Health and Safety of the children in care.
