13 | On July 23, 2026 Licensing Program Analyst (LPA) Erica Pacheco conducted an unannounced complaint inspection to provide findings regarding the above allegation. LPA met with Site Supervisor Elexa Stamps, toured the facility inside and outside and a census was taken. LPA explained and discussed the allegation and findings with Site Supervisor.
During the investigation, LPA reviewed and obtained facility records, conducted observations, and interviewed staff. Due to inconsistent statements obtained, the information did not corroborate allegation, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiencies are being cited during today’s visit. Exit interview conducted with the Elexa Stamps. Appeal rights were provided and discussed. A Notice of Site Visit was given and will be posted for 30 days.
