25 | In response to a self-reported Unusual Incident Report (UIR) dated 07/29/2026, Licensing Program Analyst (LPA), Deschampe conducted an unannounced case management - incident inspection on this date, 08/05/2026. LPA arrived at the facility at 2:15PM and was guided through the facility by the Facility Representative/Director (FR), Irene Guereca. Total enrollment is 60, with 46 children and 5 staff at the time census was taken, due to various enrollment schedules and children on vacation. A review of staff criminal records on this date 08/05/2026 indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearance or exemption and a child abuse index clearance.
On 07/29/2026, the UIR was filed with the Orange County Regional Office (OCRO) to self-report an incident from 07/28/2026. The Child Care Center (CCC) director stated Preschool Classroom 2 was out of ratio between 8:37AM - 8:39AM. Staff 1 (S1) was in the classroom with 12 children. At 8:37AM, two more children entered the classroom, with a parent who remained with S1. S1 was left with 14 children until Staff 2 (S2) arrived at 8:39AM. RP stated multiple staff called out the night before, and the facility prepared by having substitute teachers come in, but one (1) substitute teacher did not arrive until later than scheduled.
On 08/05/2026, LPA made observations, attempted interviews with children, conducted interviews with 3 staff, and obtained pertinent documentation. Due to additional information needed, the UIR needs further investigation.
Exit interview conducted and report was reviewed with the facility representative, Irene Guereca. A notice of site visit was given and must remain posted for 30 consecutive days. The “Notice of Site Visit” must be posted on, or immediately adjacent to, the interior side of the main door to the facility. Failure to post will result in Civil Penalties of $100.00. End of Report.
