| The investigation included interviews with 4 facility staff; 5 parents/guardian; 4 children; a walkthrough of the facility; observation of children’s play areas and equipment; and a review of records related to safety procedures, maintenance, and supervision practices.
Allegation: 1) Staff does not ensure day care children are provided a safe environment:
During the investigation, no hazardous conditions were observed. The indoor and outdoor areas accessible to children were free of visible safety risks. Safety measures—including staff supervision, daily inspections, and adherence to facility policies—were found to be consistent with Title 22 requirements. LPA did not observe any water on the floor.
Staff Interviews conducted and did not reveal concerns regarding staff failing to maintain a safe environment. Staff provided consistent statements indicating they routinely monitor the environment, address potential hazards, and follow established safety procedures. Staff stated children are allowed to play with water inside the classroom with staff supervision. Staff will clean the water dropped on the floor as soon as they observe it.
Allegation: 2) Staff does not ensure facility is clean and sanitized:
During the site visit, the indoor and outdoor environments, restrooms, and common-use surfaces were assessed for cleanliness and adherence to Title 22 health and safety standards.
On 4/3/26, 5/4/26 and 5/13/26, LPA observed the facility was clean, orderly, and free of accumulated dirt or debris. High-touch surfaces, equipment, and rest areas were found to be properly maintained. No evidence was found indicating that staff failed to maintain cleanliness or sanitation within the facility. No health or safety concerns were observed.
Interviewed staff stated the following: “Staff take turn to clean the children’s restrooms during children’s nap time and throughout the day. When children use the restroom, staff stand by the door and remind children to flush toilets and wash their hands. Cleaning is also done every day by janitor at night.”
Staff provided consistent statements confirming that routine cleaning and sanitizing occur daily and are documented as part of the facility’s standard procedures.
