During the course of the investigation, LPA toured the facility, conducted interviews, reviewed files, obtained records and conducted classroom observation.

Regarding allegation (1) Staff did not provide adequate supervision to a daycare child. LPA interviewed staff members. Staff members stated they have not witnessed staff not providing adequate supervision to a daycare child.

Regarding allegation (2) Staff did not properly report incidents involving a daycare child. LPA interviewed staff members. Staff members stated they have not witnessed staff not properly reporting incidents involving a daycare child.

LPA was unable to interview children due to their age and being non-verbal.

LPA interviewed parents. All interviewed parents stated they did not have any concern with facility regarding the above allegations.

Based on interviews and record review, there is insufficient evidence to corroborate the allegations: (1) Staff did not provide adequate supervision to a daycare child. (2) Staff did not properly report incidents involving a daycare child. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur in the day care facility, therefore the allegations are Unsubstantiated.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Nicole Lorenzo.
