13 | Licensing Program Analyst (LPA) Cynthia Sun conducted an initial 10 day investigation regarding the above allegations. LPA Sun met with licensee Mayuri Soneji, who accompanied the LPA on a tour of the facility. At 8:30 AM LPA observed no children in facility. By 9:36 AM LPA observed 1 infant, 10 preschool children, and 4 school age children with 1 staff present at the facility.
A review of staff records on this date 7/31/2024 indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 7/24/2024, Orange County Regional Office received a complaint alleging the facility is operating out of ratio and over capacity. The Reporting Party (RP) stated there was 1 staff supervising 15-20 children.
During today’s inspection, LPA Sun obtained facility roster, and interviewed 1 staff. Licensee acknowledged that they are over ratio and providing care for 15 children during today’s visit. Based on LPA observation, staff interview, and record review the preponderance of evidence standard has been met, therefore the above allegation the facility is operating out of ratio and over capacity is found to be SUBSTANTIATED. California Code of Regulations, Title 22 is being cited on the attached LIC 9099D.
Due to the Type A violations cited today, the licensee shall post, and provide copies, of the report to parents/guardians of the children in care at the facility by the next business day, and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. The licensee is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child’s file. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days.
