Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection for the purpose of amending a report issued on 9/24/2020. The Covid-19 Emergency Response questionnaire was reviewed and answered by the licensee, Sara Naz Yazdani. LPA and the licensee's assistant, toured the facility, and the floor and yard plan (LIC 999) were verified. Census was taken. The overall census observed were 3 children of whom all were over 2 years old months old. Facility hours are 8: am - 5: pm., Monday through Friday.
A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Notice of Site Visit was posted. The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The facility representative was provided a copy of their appeal right (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. First level appeal is to Regional Manager; address is above on the report.
