LPA reviewed Facility Personnel Report Summary for this date and did not find, Staff Elena Maslenkina and 1, Room mate # Jin won Kim. A citation will be issued under case management visit for today's date.
The Regional Office received a complaint on 06/15/2023 alleging that Due to lack of supervision, a child left the day care and ran into the street. It is noted that the "street" in question is an offshoot from Darby that contains 6 homes (3 on each side) with large drive in between . During today's visit, LPA observed all children were dropped off at the back of the home at the patio doors, not the gates. Parents must go through two gates to get to the back patio per staff. LPA observed that both assistants accepted children into care after arriving through the patio doors not the gates. Interview with Ms. Tangeland indicated she accepts the children into care through the patio door not the gates. Interview with licensee via telephone indicated he was aware of an incident that a neighbor was concerned because a parent had not been watching their child and ran around car but child had not been accepted into care yet. Interview with Ms. Tangeland who stated that last week, as she was at the front door speaking with neighbor regarding concerns of a child leaving the gates and going into drive where cars drive through the child in question was being dropped off at the patio by their parent. Ms. Tangeland further stated parents are advised to park on Darby and walk to facility. LPA interviewed a parents who indicated drop off is at the patio and not the gates. There are two gates. One gate is from the drive and spans the length of the home, the second gate is a half gate that leads into back yard and patio door.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
An exit interview was conducted with Assistant Ms. Tangeland, Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058) 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the Regional Manager to the address listed.
