During LPA’s first visit to the facility, LPA observed at least one qualified teacher in every classroom during naptime. The second time LPA was at the facility LPA observed the children to be under the direct supervision of qualified teachers.

Regarding the allegation, Facility has cockroaches. 7 out of 7 staff denied there are cockroaches in the facility. Staff #7 (S7) stated that staff maintain the facility very clean and S7 has staff clean throughout the day and deep cleaning is done by a cleaning crew. 5 out of 5 children did not make any disclosures about the above allegation. 3 out of 3 parents interviewed did not make any disclosures regarding the allegation.

Regarding the allegation, Unqualified staff left alone to supervise children. 7 out of 7 staff denied unqualified staff are left alone with children. 5 out of 5 children did not make disclosures regarding the allegation. 3 out 3 parents interviewed made no disclosures regarding the allegation.

During the course of the investigation LPA took pictures of the kitchen and obtained a copy of the Children’s Roster and Personnel Report. The Reporting Party (RP) provided a video showing a cockroach in a cabinet, the video is zoomed in so it is unclear if the video was taken at the facility. Additionally, the video did not have a time stamp, LPA requested the original video with a time stamp included and LPA did not receive a response. Therefore, the video provided does not sufficiently support the allegation.

Based on LPA’s interviews and record review, there is insufficient evidence to corroborate the allegation of Unqualified staff left alone to supervise children, Facility has cockroaches and Staff leave children alone during nap time. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

Exit interview was conducted, and report was reviewed and discussed with Director Maria Elena Rivera and Licensee, Miriam Salcedo. Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. The facility was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.
