32 | Regarding the allegation of “Staff are not properly dispose of broken toys.”
During the physical plant inspection on 05/06/2026, LPA observed the facility stored the big broken toys behind the locked gate, not in the shower.
During the staff interview, 5 out of 5 staff denied the allegation of “Staff are not properly dispose of broken toys.” The staff #1 (S1), the staff #2 (S2), the staff #3 (S3), the staff #4 (S4), and the staff #5 (S5) stated that they threw away the broken toys in the big dumpster located outside of the facility; if the toys are too big, they store them behind the locked gate, take a picture and send it to the director. During the children interview, all 5 interviewed children did not disclose any information pertaining to the allegations.
During the parent interview, all 5 interviewed parents did not divulge any information pertaining to the allegations or express any concerns regarding care of the children. After reviewing the nine photos provided by the reporting party, the LPA did not find observable evidence to support the allegations at this time.
Based on LPA’s observations, interviews, and records review, the preponderance evidence of 1) Staff yells at day care children, 2) Staff are not adequately supervising day care children, 3) Staff did not ensure the washing machine was not in disrepair, 4) Staff are not following napping equipment procedures, 5) Staff did not follow daily inspection for illness protocols, 6) Staff did not ensure they didn't have stagnant water resulting in mosquitos, 7) Staff are not properly disposing of diapers, 8) Staff are not properly disinfecting potty training toilet, and 9) Staff are not properly dispose of broken toys has not been met. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegations are Unsubstantiated.
