Reporting Party (RP) stated a child in the facility gained access to a knife and was witnessed running towards other children screaming on 05/05/2026 in the kitchen of the facility.

During the course of investigation, LPA interviewed 3 staff members, 2 parents and 4 children.

Regarding the allegation, Staff did not ensure sharp objects were locked and inaccessible to children, three out of three interviewed staff denied a child was running around with a knife toward other children. All staff denied sharp objects were accessible to children. Staff #1 (S1) stated there are no knives in the classrooms; snacks do not require cutting; lunch is cut in the kitchen and brought to classrooms already prepared. S1 denied children enter the kitchen and stated harmful items are never left out. S1 reported that at around 4pm during free play, Child #1 (C1) was using playdough tools, including a plastic playdough knife. S1 described C1 as active and loud but stated C1 was not running toward anyone. A school-age child #2 (C2) said “C1 has a knife” and “C1’s going kill,” which S1 stated was untrue. S1 stated other children were never in danger and later addressed the false accusation with the school-age group. Staff #2 (S2) denied knives are ever accessible and stated snacks and meals arrive ready to serve with no cutting needed. S2 denied knowing of any incident involving C1 and stated S2 is not often with C1. Staff #3 (S3) denied any dangerous items are accessible and stated knives are never in classrooms. S3 described C1 as sweet and very active and stated S1 sometimes assists when C2 is having difficulty.

Four parents were contacted for interviews, and LPA was able to interview two. Two out of two parents interviewed had no issues or concerns with the daycare. Four out of four children interviewed made no disclosures about the above allegation.

LPA’s observations during of the investigation, LPA thoroughly inspected the kitchen and classrooms, LPA did not observe anything sharp or dangerous accessible to children. The kitchen was clean and nothing was left out that could pose a danger. LPA did not observe any knives in the classroom.

Regarding the allegation, Staff did not ensure adequate supervision was provided to children, three out of three interviewed staff denied children are not supervised at all times. S1 stated staff ensure the supervision of children by always maintaining ratios; staff don’t put their backs toward the kids so staff can always see them. S1 stated that Staff are always interacting with the children. S1 denied a child had access to a knife due to a lack of supervision by staff. S2 stated S2 maintains the supervision of children by always being right there, S2 never leaves the children alone. S2 keeps eyes on them at all times, S2 treats the children well as if they were S2’s own kids. S2 denied leaving children unsupervised, creating an opportunity for a child to grab a knife. S2 stated not keeping anything that could be dangerous within the children’s reach. S2 never puts the children near any danger. S3 stated staff ensure the supervision of children by keeping S3’s eyes always on the children, they are never left unattended. If S3 ever needs to go to the restroom or something like that S3 will ask another teacher to step in.Two of two parents interviewed made no disclosures regarding the allegation. Four out of four children made no disclosures about the above allegation.

LPA’s observations during the inspection, while touring the facility, LPA Valdez Santana observed the facility to be within ratios and children were being supervised at all times.

During the course of the investigation LPA took pictures of the kitchen and obtained a copy of the Children’s Roster and Personnel Report.

Based on LPA’s interviews, observations and record review, there is insufficient evidence to corroborate the allegations of Staff did not ensure sharp objects were locked and inaccessible to children and Staff did not ensure adequate supervision was provided to children. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.

Exit interview was conducted, and report was reviewed and discussed with Director Amanda Gonzalez. Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. The facility was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. End of Report.
