LPA observed Room#3 infant room has a wooden handlebar fixture installed on a wall, a low-rise toy bin in the same room and foam gym. During staff interviews, multiple staff stated they have witnessed children hitting their heads on the handlebar and the low-rise toy bin resulting in injuries. Staff also mentioned witnessing children hitting their heads on the wall while using the foam gym. Director stated they are requesting padding for the handlebar and the toy bin.
