Play equipment is not safe to children's use and staff does not ensure facility pavement is in safe condition.
During the investigation, LPA Habtom conducted interviews and observations regarding the allegations that the play equipment is not safe for children’s use and staff do not ensure the facility pavement is in safe condition. LPA conducted observation and was unable to locate the age range for the play equipment on the playground set. Interviews also did not provide further details about the safety of the pavement and the playground set. The playground is shared with transitional kindergarten and kindergarten children at the elementary school. Concerns were raised by staff on the type of shoes worn outside while playing. Based on the limited information collected from interviews and observations, LPA was unable to validate or invalidate that the play equipment and pavement is not safe therefore the outcome is UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegations did or did not occur.
