At 2:40pm on 3/2/23, Licensing Program Analyst (LPA) Matthew Gallo met with Licensee Jodie Radakovitz for the purpose of an unannounced required 1-year inspection. Licensee states no others are present in the home today except for her two adult sons, who were present for the inspection. At 2:45pm, LPA observed a total census of 4 preschool children.

Licensee guided LPA on a tour of the facility, and a health and safety inspection was conducted in all areas accessible to children. Prior to inspection, the off-limits areas included the Master Bathroom, Laundry Room, Sons' Bedrooms, and Garage. During inspection, licensee requested that Master Bathroom be moved to an on-limit area and LPA carried out inspection to ensure space met all licensing requirements. Moving forward, the Master Bathroom will be on-limits. Licensee acknowledged that children may never enter the remaining off-limit areas.

LPA observed the required postings, a working phone, 2A10BC fire extinguisher, and functioning smoke and carbon monoxide detectors. Firearms are appropriate stored with ammo in separate locked containers. Property has a pool that is properly gated. Toxic and hazardous items are inaccessible to children. Fireplace waiver is on file. There are no stairs in the home.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed 4 children’s files which were observed to be complete, including 15 minute sleep logs for enrolled infant.

Report continues on LIC 809C
