On August 14, 2026, at 09:31 AM, Licensing Program Analyst (LPA) Courtnee Peebles conducted an unannounced visit to Discovery Isle Children’s Center of Temecula (CCC) to deliver the investigative findings. LPA met with Director Tonia Goldbach and discussed the results of the investigation.

The complaint received on July 23, 2026, alleged that a child was left unattended. Specifically, it was reported that a child (C1) was locked in an outside bathroom without supervision. Interviews revealed that the classroom transitioned outdoors for play at approximately 4:30PM and returned indoors at approximately 5:05 PM. However, C1 was not brought inside with the group and remained unattended in an outdoor restroom. At approximately 5:45 PM, C1’s guardian arrived at the facility to pick up the child. Staff were unable to locate C1 and acknowledged that they did not know where the child was. Staff initiated a search of the facility and ultimately located C1 inside a locked outdoor restroom at 6:05 PM. C1 had been unattended and separated from the classroom for approximately 50 minutes. It is also noted that the outdoor temperature at the time of the incident was approximately 95 degrees Fahrenheit,

creating an additional safety concern. After the incident that occurred on 07/22/2026 , Director (D) has made corrective measures by doing names to faces every thirty minutes and supervising transitions from outdoors to indoors and vice versa. Training documents have been to sent to the department for proof and Directero (D) has since ensured all classrooms, restrooms, and spaces are checked thoroughly before closure.

Based on interviews conducted and records reviewed, the preponderance of evidence standard has been met. Therefore, the allegation that a child was left unattended is substantiated. California Code of Regulations, Title 22, 101229(a)(1), is being cited on the attached LIC 9099D.

The facility representative shall post and provide copies of the Type A report to all parents/guardians of children currently enrolled at the facility by the next business day. Copies of the Type A report must also be provided parents/guardians of newly enrolled children for the next 12 months. The licensee must retain a copy of the Acknowledgement Receipt (LIC 9224), signed by the parents, in each child’s file.

An exit interview was conducted, and Appeal Rights were provided. A Notice of Site Visit was issued and must remain posted for 30 consecutive days. The licensee was reminded that the Notice of Site Visit must be posted in a prominent location, visible to parents and caregivers without request. Failure to post the notice will result in civil penalties of $100.
