13 | On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido conducted a subsequent complaint investigation to deliver final findings. An initial visit was conducted on 06/08/26 at which time LPA conducted interviews and reviewed records. LPA met with Licensee, Renee Williams; toured the facility and took a census.
During the investigation, LPA reviewed records and interviewed pertinent parties, including facility staff and children.
It was alleged the licensee did not provide parent rights form. Licensee stated they attempted to give parental rights to the guardian but did not ensure documents were received. Other interviews reported having access and prior knowledge of parent rights information. Additionally, LPA observed at front entry required posting of parent rights form. During the investigation, LPA determined the missing documentation poses no health and safety risk to children in care and did not affect the overall operation of the facility. This is considered a technical violation, and no citation is being issued currently.
