On October 5,2023, at 11:30 AM Licensing Program Analyst (LPA) Courtnee Peebles arrived at the facility to conduct an annual inspection as part of a compliance review. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed:

· Normal days and hours of operation are: Monday through Friday 07:30 AM to 05:00 PM

Staff is unaware of how long the licensee has been absent from the facility, however LPA was made aware that licensee will be back by October 13 2023. LPA reminded assistant that licensee must be present for 80 percent of the time and due to the licensees daily absence that 80 percent from licensee is not being fulfilled. A citation will be issued.

· A working telephone is present, and the current phone number is on file

· A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection.
