32 | It was revealed that the facility transfers children between classrooms so frequently that the child did not know which classroom he was supposed to nap in and got lost between the classrooms. The child's supervising teachers, T1 and T2, did not notice that the child had wondered into a different classroom. It was alleged that staff members did not ensure that the facility restrooms were kept clean. On 08/12/2026, LPA observed the preschool bathrooms to be unclean: dirty toilets, dirty hand washing sinks, dirty floors, and debris on the floors. Interviews with staff have revealed that staff members are not provided adequate cleaning time that is not also supervision times of the day. Interviews with parents revealed a concern for the sanitation of the facility and the correlation to illness in the facility.
Based on interviews, file reviews, and observations conducted the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. 1 Type-B deficiency and 2 Type-A deficiencies and 1 Type-B deficiency were cited on subsequent 9099-D pages. D1 acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099-D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee.
An exit interview was conducted, and the report was reviewed with Director DiPippo. LPA provided Licensee Appeal Rights to D1.
